Personal Data :
Patients:

Assisting Nature IVF & Genetics collects data to operate effectively and provide you the best Reproductive Services. The data are collected for specified, explicit and legitimate purpose and are not further processed in way incompatible with this purpose. When you undergo fertility treatment with a partner, information about you and your partner is recorded in a joint patient case file on our Assisting Nature IVF & Genetics database and either partner may (subject to any relevant exemptions under privacy laws) have access to this case file. The data is retained in a form which permits the identification of the information only for the time required for the processing of personal data.
You provide some of this data directly, such as when we create a new registration form or start a new treatment (for example, when we interact with you in person, when you complete consent forms, by email, over the phone, via text message, multimedia message, or when you interact with us via our website or on our social media platforms).
In some cases, it may be necessary to collect your personal information from another person such as your referring doctor, other healthcare practitioners involved in your care, Medicare, your private health insurer, your partner, relatives and next-of-kin.

We also obtain data from third parties. We protect data obtained from third parties according to the practices described in this statement, plus any additional restrictions imposed by the source of the data. These third-party sources vary over time, but include:

  • Social networks when you grant permission to an Assisting Nature IVF & Genetics employee.
  • Partners with whom we offer co-branded services or engage in joint marketing activities, such as Microbiological Laboratories, Genetic Units.
  • Publicly-available sources such as open government databases or other data in the public domain.

Providing accurate and complete information is very important for the safety, quality and effectiveness of the fertility treatments we provide. If you do not provide accurate and complete information, or you withhold information, it may impact upon our ability to provide you safe and effective services. If you have any concerns about personal information we have asked you to provide, please share those concerns with us.
You have choices about the data we collect. When you are asked to provide personal data, you may decline. But if you choose not to provide data that is necessary for your treatment, you may not be able to use our services.

The data we collect depends on the context of your treatment, the choices you make, including your privacy settings. The data we collect can include the following:

  • Name and contact data. We collect your first and last name, identification card number/passport No, email address, residence address, phone number, and other similar contact data. We may also collect personal photos.
  • Your medical history data, test results, medical records, diagnostic imaging, photographic films, family medical history, investigations, treatment history and advice, and other information required for your fertility treatment.
  • Demographic data. We collect data about you such as your age, gender, country, and preferred language.
  • Financial data. Depending on your financial settlements, we may also collect information about your Medicare eligibility (including your Medicare number), any private health insurance coverage you may have and any eligibility for benefits or compensation from another source.

We also collect information you provide to us and the content of messages you send to us, such as feedback, or questions and information you provide for customer support.

Generally, every time you make contact with Assisting Nature IVF & Genetics, new information is added to your record. Please let our staff know if your data have changed since your last visit.

 

Other individuals:
We may collect information about individuals who are not patients of Assisting Nature IVF & Genetics. This includes individuals making enquiries about our services, such as our Patient Information Seminars, events, via our website or social media platforms, or the emergency contact data and family history collected from a patient about another individual.

GPs, referring doctors and other healthcare professionals:
We may collect personal information about health practitioners who interact with us (such as referring doctors or other healthcare practitioners involved in the care of our patients). This is typically information such as name, contact details, professional details and information regarding the interactions with Assisting Nature IVF & Genetics. This information is collected for the purpose of patient care and the management/operation of Assisting Nature IVF & Genetics.

We may also collect personal information about individuals who we deal with on a commercial basis such as suppliers, contractors and individuals in organizations to which we provide goods and services or from which we acquire goods and services. We may collect personal information including name, position, contact details, license or registration number, ABN, bank details and other information relevant to the capacity in which you are dealing with us. We generally collect this information to enable us to contact and deal with you and to enable us to fulfill any obligation or undertaking relevant to your relationship with us.
